Step 1) Submit New Patient Forms
Our technology will guide you through the registration process using any device. This process is secure, prevents data errors, and you won’t have to arrive 15 minutes early for your appointment! It is critical you fill these forms out in their entirety. If you have a medication list or medical history you would like to submit separately, please click here.
Step 2) Send us your insurance card, and medication list, if you have any, so we can place it in your record and verify your coverage before your appointment. Please note we need both sides of the card! Insurance verification is a time consuming process and it is imperative we have your card in order to do so. Alternately, you can take a picture with your smartphone and email it to email@example.com. Don’t forget to email both sides!

Step 3) Schedule Your First Appointment
New patients may schedule their first appointment using the button below. We may call you if we have any questions, in particular when we are verifying insurance benefits. You will receive a request to confirm your appointment via via text, email or phone 24-48 hours prior to your appointment. Please note, for appointments within 24 hours, please call the office at 772-774-7787 to schedule. You should still fill out your intake forms before you call schedule, if you can.
